Ominvisor™ is a 4th generation disease management hardware and software system that enables case or disease managers and clinicians at your facility to receive information about patients’ daily vital signs and symptoms.
Each day, your patients have their vital signs remotely monitored by a “Health Check” at home using their Commander. Daily biometric weight and symptom data are transmitted and analysed by the Ominvisor™ System in your office. Omnivisor™ automatically identifies and alerts you about “exception” patients with symptoms outside of the parameters set by your clinical management team or the patient’s physician.
Cardiocom’s “management by exception” approach lets you know when the patient is outside of the established parameters, thereby allowing you to focus your attention only on patients who need an adjustment to their care plan. Omnivisor™ allows the case manager to assist the treating physician by providing current, objective, biometric information about a patient. Management adjustments can be initiated in a timely manner, thereby preventing further deterioration of the patient’s condition.
Omnivisor’s powerful clinical management system also includes patient demographics, healthcare and emergency contacts, medications, blood chemistries, functional labs, clinical notes, daily objective monitoring parameters and symptom data. All monitoring parameters are fully programmable. The system can be customised to integrate and exchange data with most customers’ central data warehouse or clinical management program.
