Care Companion’s Long Term Benefits for Newham

Newham University Hospital NHS Trust introduced an innovative telemonitoring service for patients seen by nurses from the Long Term Conditions Programme.

The patients had congestive heart failure and/or obstructive pulmonary disease, and the initiative had several aims:

  • To help patients stay healthy
  • To keep patients out of hospital
  • To proactively monitor patients’ health at home and adjust care and medication accordingly
  • To reduce unplanned hospital visits
  • To increase clinical efficiency
  • To reduce unnecessary home visits by nurses

All of the aims have been achieved.

The home care unit chosen after thorough research was the Care Companion.

Programme Manager Angela Single, said: “It provides in-depth collection, assessment and review of patient information and works in seamless conjunction
with the care provider.? Patients welcomed the programme and asked to be part of it. The reasons, in the words of one: “I am grateful to have Care Companion in my home because my nurse and GP receive my vital signs and assessments every day helping me to stay out of A&E and in my own home.?
